My View and Philosophy on Nose Aesthetics

Rhinoplasty is the most performed plastic surgery all over the world. As the number of people undergoing surgery increases, people's faces are becoming more and more similar to each other. Because surgeons usually standardize the same technique and apply the same technique to each patient. This results in a standard “copy and paste nose” and a standard facial appearance.

The noses I design and make are not standard noses, but noses specific to the person and needs. Because from my point of view, I evaluate the social environment of the person and the harmony with the face as a whole in nose design. If I give an example; I do not make a curved nose for a 50-year-old male patient. For this reason, for the noses I design and make, my patients' friends say “There is a change in you, but I cannot figure out what it is” instead of saying “Your nose looks very good”. My motto is: “Change without alienating yourself”.

Open Rhinoplasty

Open technique rhinoplasty is a surgical technique based on the understanding of stripping the nasal skin from the underlying cartilage and bone tissues after making an incision in the skin just below the tip of the nose. Compared to closed rhinoplasty surgery, tissue trauma is slightly higher. For this reason, since the edema will be more, the visual recovery takes a little longer.

The most important advantage of rhinoplasty surgery performed with the open technique is that it provides a great view and thus offers the surgeon a much better maneuverability. The surgeon works more comfortably in open rhinoplasty, can knot the sutures more easily, and the instruments are much easier to enter and exit the nose. Since the surgeon will not get into difficult positions to try to see through the tunnels, the surgeon's posture and spine health will not deteriorate.

When all these situations are evaluated together, it is not right to leave the choice of “open rhinoplasty surgery or closed rhinoplasty surgery” to the patient alone. It is a personal and situation-specific decision that the surgeon and the patient should decide jointly.

Another option about Open or Closed may be the following. If the surgery is started closed and the structure and anatomy of the nose do not allow it, it is necessary to consider the possibility of returning to open rhinoplasty during surgery and not to avoid making this decision if necessary.

Closed Rhinoplasty

The biggest difference of closed rhinoplasty from classical open technique rhinoplasty is that there is no incision visible from the outside under the tip of the nose. Thus, nose surgery is performed with tunnels opened through the nose in the closed technique. The incisions and repair stitches remain hidden in the nostrils and dissolvable stitches are used, so it is not necessary to remove these stitches.

How is Closed Rhinoplasty decided?

When deciding on Closed Rhinoplasty surgery, we decide by looking at the patient's bone, cartilage and skin structure. For example, it will be very challenging to intervene in deformed noses and in the nose of patients with advanced curvature in the bone-cartilage complex with the closed technique. This difficulty can torment both the patient and the physician.

The most suitable patients for the closed technique are patients with more nasal dorsum and nasal tip problems and patients with mild arches.
